Health Management staff availableUPMC Health Plan offers health management services for our members who have asthma, COPD, diabetes, or cardiovascular disease (including conditions such as hypertension, coronary artery disease, hyperlipidemia, and heart failure). Through telephone contact and educational information, the Health Plan’s care managers support members with these chronic conditions. To reach our Health Management Department, call 1-866-778-6073. If you have patients who are UPMC Health Plan members and who have any of the conditions mentioned above, let them know there is help available from our Medical Management Department. If appropriate, Medical Management can assign your patients a care management nurse (and/or social worker). The care manager will help your patients assess, plan, implement, coordinate, and monitor their treatment and services. Essentially, this case manager will be an advocate for your patients by identifying appropriate resources and support groups. In addition to increasing your patients’ knowledge of their condition and proper treatment plan, care managers can coordinate care to eliminate duplication or inappropriate use of resources. Patient participation in this program is voluntary. |
