Redefining health care: The UPMC Health Plan Partners Program

A primary care physician’s commitment to his or her patients is something that will never change, even in the face of constant change in the health care industry. Strong partnerships with our network physicians and hospitals enable us to improve clinical outcomes and the health of our communities. They are essential to providing our members with the high-quality service they have come to expect. That’s why we’ve developed the Partners Program, an initiative that consists of Health Plan programs and offerings that focus on the wellness of the patient.

The principles of the Partners Program are consistent with those of the Patient-Centered Medical Home Model, which aims to strengthen the physician-patient relationship by encouraging coordinated care and guiding patients to make more informed health care choices. This model places the patient and physician at the center of care, with supporting roles and division of labor among the family, the community, and the Health Plan to more effectively meet the needs of patients and members.

Primary care physicians participating in the UPMC Health Plan Partners Program will enjoy benefits such as access to enhanced tools, materials, and services to support patient care and through improved, actionable data.
