Our Health Management program works for your employees

Employees with chronic conditions such as diabetes, respiratory illnesses, and cardiovascular disease, may benefit if they participate in UPMC Health Plan’s Health Management programs.

Members may enroll themselves in the program, or they may be enrolled automatically through claims data and predictive software or by their physician. Members are grouped into low, moderate, or acute categories, depending on the type and intensity of the intervention needed.

Interventions range from educational mailings and access to online tools to weekly phone calls from a registered nurse.

Your employees will benefit from the Health Management Department’s team approach to health care. The team collaborates with physicians to help members delay progression of their condition or disease. The team also helps identify symptoms that could signal a health problem and then recommends treatment or a referral for those issues. The program emphasizes self-care and the doctor-patient relationship.

The goal of all of the Health Management programs is to improve quality of life, which makes people happier at home and more productive on the job. Our health Management programs help members get the care they need to be as healthy as possible.
