 Corporate wellness questionnaire
Is your company’s culture ready for health promotion? Our corporate wellness questionnaire can provide you with essential information before you begin to plan your worksite wellness program. The survey identifies worksite features that may support health promotion activities or that may be barriers to good health. Examples are stairwell access, on-site cafeterias, and the content of vending machines. Our 20-minute comprehensive online health risk assessment, called the MyHealth Questionnaire, allows your employees to build a personalized health profile. It will give them a health score and an analysis of their individual health risks and family history and will compare their results to others in their age range. It also generates a personal action plan, including ways employees can reduce their health risks. After completing the questionnaire, employees may explore the many free online tools and resources available on the MyHealth OnLine portal. |
