The National Health Care Anti-Fraud Association (NHCAA) estimates between 3 and 10 percent of health care claims submitted to insurance companies are fraudulent. Such claims add up to between $70 and $300 billion dollars per year. Insurance fraud can be committed any number of ways by members and providers.
