Heart disease — Are you at risk?


The American Heart Association has identified several factors that increase the risk of coronary heart disease and heart attack. While you cannot change certain risk factors, there are others that are under your control. The more risk factors you have, the greater your chance of developing heart disease.

UPMC Health Plan offers a Cardiovascular Health Management program for members who have been diagnosed with heart disease. A personal care manager provides information specific to each member’s condition, and is the member’s link to support in the management of his or her heart disease. A personal care manager works closely with the member’s own physicians, as well as Health Plan pharmacists, nurses, and social workers to make sure each member has the information and support to live well with his or her condition.
