UPMC for Life Specialty Plan

Did you know that UPMC for Life Specialty Plan has a complex case management program to help members who have serious medical or behavioral health conditions?

It can be hard to remember everything your doctor told you to do when you have several health conditions. UPMC for Life Specialty Plan has nurses and social workers who can help you and your family. The case management staff can help:

• Coordinate appointments with the providers.
  
• Provide information about your medical or behavioral health conditions.
  
• Help you and your family develop a plan to better manage your health care.
  
• Link you to community services.

Enrollment in the complex case management program is voluntary. If one of our case managers calls you to explain the program, you can agree to enroll or say that you don’t want to enroll.

If you enroll and find at a later time you no longer want to participate, you can tell us at that time.

If you are a UPMC for Life Specialty Plan member and would like more information on the complex case management program, you can call us at 1-800-606-8648 and select option 4 for case management. TTY users should call 1-866-407-8762. Case management staff is available Monday through Friday from 8:30 a.m. to 5 p.m.
