Life can be hard for people who have an illness or a chronic condition. Difficult life situations can make illness worse. UPMC Health Plan ’s Special Needs staff can help.

Our Special Needs staff works with members who have illnesses or chronic conditions. We can give you useful information. We can help you find a doctor and make an appointment. We can help you decide what questions to ask your doctor at your next visit.

We can connect you with resources. A group in your own community might offer just what you need. Our staff can put you in touch with other departments so you can:

• Talk to someone in the Pharmacy Department about medications.
  
• Sign up for the UPMC for a New Beginning maternity program at no additional cost.
  
• Talk to one of our nurses if you have asthma, diabetes, or heart disease.
  
• Get a referral to a specialist who can help you with addiction or depression.
