What to do if You Forget Your Password
- On the Login screen, click
on the Forgot your Password? link.

The Get Password
screen will be displayed prompting you to enter your User Name.
If you've forgotten your User Name, click on the Forgot your
User Name? link.

Enter the
User ID that you have been assigned and then click on Submit.
The second Get Password screen will be displayed.
The date entry
requirements for each field are detailed below.

four digits of your social security number.

Enter your
5-digit zip code.

would enter 04 in the first box, 01 in the second box, and 1960 in the last

This question
will vary depending on the Secret Question and Answer you selected when you
originally signed up for Health Plan OnLine.

Email Address
|
Enter your email
address.

- When you have completed the
form, click on Submit. If you would like to clear the form and
begin again, click on Clear. After clicking on Submit,
the following message will be displayed:
- At the email address
provided, you will receive a message like the one shown below:

Click on the
link in the email message. The Change Password screen will be
displayed:
