What to do if You Forget Your User ID

 

  1. On the Login screen, click on the Forgot User ID? link.

  1. You will be prompted to select your User Type.  From the drop-down menu, select UPMC Health Plan Member and then click on Submit.

  1. The Get User Name screen will be displayed.

The data entry requirements for each field are detailed below.

Field

Description

Last 4 digits of SSN

Enter the last four digits of your social security number.

Date of Birth

Enter your date of birth in mm-dd-yyyy format (e.g., if you birth date is April 1, 1960, you would enter 04 in the first box, 01 in the second box, and 1960 in the last box).

Zip Code

Enter your 5-digit zip code.

Email Address

Enter your email address.

 

  1. When you have completed the form, click on Submit.  If you would like to clear the form and begin again, click on Clear.

  2. After clicking on Submit, the following message will be displayed:
