UPMC Health Plan Urges Members with Diabetes to "Take Control" PITTSBURGH, PA (September 19, 2003) — "Take Control," a handy guide to healthy living with diabetes, is now available through UPMC Health Plan's Diabetes Health Management Program. "Take Control" is a trifold pamphlet that contains tips for people who live with diabetes and describes some of the ways the Diabetes Health Management Program works with members, including scheduled phone calls from staff nurses and reminders about tests and examinations. This no-cost, voluntary program is a supplement to the personal plan of care developed by the member's doctor. The pamphlet also provides a checklist of things people with diabetes can do on a daily, trimonthly, and annual basis to better manage their disease. More than 17 million people in the United States have diabetes, and each year more than 800,000 people discover they have the disease. Pennsylvania has the 11th-highest instance of diabetes in the country. UPMC Health Plan provides enhanced-access HMO and other health benefits products to more than 400,000 members.
