Members can get one-on-one support from UPMC for Life health coaches who work with you to help you manage chronic medical conditions, improve your health, and enjoy the best quality of life possible.
Our health management programs have been developed in collaboration with leading clinical experts at the University of Pittsburgh Medical Center, using the latest research and guidelines. Health coaches are available to help identify, plan, and achieve the lifestyle changes that are important to help our members have the best quality of life.
UPMC Health Plan's integrated team of health care professionals – physicians, nurses, social workers, and pharmacists – provide additional support to help members accomplish their personal goals.
We currently offer care management programs for:
- Asthma
- Coronary artery disease
- Chronic obstructive pumonary disease (COPD)
- Diabetes

- Heart failure
- High blood pressure
- Wound care treatment
