That’s why we’ve developed the Partners Program, an initiative that consists of programs and offerings that focus on the wellness of your Health Plan patients.

The principles of the Partners Program are consistent with those of the Patient-Centered Medical Home Model, which aims to strengthen the doctor-patient relationship by encouraging coordinated care and guiding patients to make more informed health care choices.

The Partners Program includes:

• Physician-directed multi-disciplinary care teams
• Rewards for physician and members for achievements in care
• Community resources for health related events
• State-of-the art online tools including provider reporting
• Increased recognition for participating physicians
• High-touch service with Physician Account Executives

As a Partners Program physician, you will receive many support services from UPMC Health Plan. These include:

• Eligibility for quality incentive reward program (also required to have 200 or more
  Health Plan patients)*
• Practice Rewards Program
• Physician reports with actionable data to better manage lifestyle risks and
  chronic disease
• Access to UPMC Health Plan Tobacco Cessation Program and Health/Disease Management Programs
  for your Health Plan patients
• Physician Account Executives, who serve as a critical link between you and the Health Plan to coordinate
  and provide resources that focus on improvements in the overall health of patients
• Member Advocates to assist in the coordination of community resources for your Health Plan patients
• A commitment to work with you to improve the validation of member data and develop innovative techniques
  to collect and report non-claims-based data.

*UPMC for You (our Medical Assistance program) has a separate incentive reward program for CY 2008.
