UPMC Health Plan's cardiovascular health management program includes individual programs for coronary artery disease, heart failure, hypertension, and hyperlipidemia (high cholesterol). Consider these national statistics:

  • About 14 million Americans have coronary artery disease, making it the largest killer of both men and women in the United States.
  • Nearly five million Americans live with heart failure.
  • Hypertension is the most common primary diagnosis in the United States.

If left untreated, any of these chronic conditions can lead to serious health problems, even death.

The good news: With medication and lifestyle changes, you can learn how to not just live with a cardiovascular condition, but to live well.


Program Overview

Our cardiovascular program strives to ensure that members with cardiovascular conditions have regular physician visits, consistently use prescribed medication, and make the necessary lifestyle changes to keep their condition in control.

Enrolled members receive reminders for annual checkups, the MyHealth Insight Newsletter, and information about how to access MyHealth OnLine, our 24/7 online member resource. Through this online self-service center, members have access to health-related educational materials, resources, and interactive tools to help them manage their condition.

If your condition requires it, you will receive all of the benefits listed above, plus monthly, weekly, or as-needed telephone calls from your personal care manager to monitor your progress. You will also receive additional educational materials, a welcome packet, a quality-of-life survey, and a satisfaction survey.
