The Notice of Privacy Practices provides full disclosure of how UPMC
Health Plan
may use and disclose your personal health information (PHI) and how
you can access
this information.
The Personal Representative Designation Form is used when you want
to appoint
a personal representative to act on your behalf in discussing your health
and
benefit coverage.
The Member Authorization to Use/Disclose Protected Health Information form is used if you want to indicate that UPMC Health Plan/UPMC Health Benefits may use or disclose your protected health information (PHI) that it has received, collected, or maintained in its files for certain purposes that – according to federal law – require your written permission.
