Veterans Breakfast Club will talk about its program and how you can share your stories with the community. You’ll also learn about community organizations that provide resources and support to veterans and their caregivers. How to participate on the day of the event: Call 1-855-962-1209 (no PIN required). It is not your Service Coordinator’s choice. • Your Service Coordinator must be wearing a mask when they come to your home, even if they are vaccinated against COVID-19. If you have questions about your options for assessments conducted by your Service Coordinator, please call the Office of Long- Term Living Participant Line at 1-800-757- 5042 (TTY: 711) Monday through Friday from 9 a.m. to noon and 1 p.m. to 4 p.m. Home- and Community-Based Services Assessments If you receive Home- and Community-Based Services, your Service Coordinator must assess your needs every year, and sometimes more often. You have options for how you can be assessed while staying safe from COVID-19.
