CARING FOR LIFE • Issue Three • 2021 8 Preventive Care Eligibility Requirements Diabetic retinal eye exam (only if diabetic) Who: Ages 18-75 and have a diabetes diagnosis from your doctor When: 1-2 years, depending on doctor recommendation Mammogram Who: Ages 52-74, female, no prior mastectomy When: Every 27 months Colorectal cancer screening (Cologuard®, colon FIT kit, colonoscopy, or sigmoidoscopy) Who: Ages 51-75. You may need a prescription from your doctor for this screening. When: 1-10 years based on your chosen screening method For questions about the preventive care rewards program or to see if you’re eligible for rewards this year, please call your Health Care Concierge team at the number on your member ID card. Finish this year strong by getting your needed preventive care by Dec. 31! More than 75,000 rewards were earned by UPMC for Life members for getting preventive care this year. As a reminder, preventive care is free when you see an in-network provider! * Who can earn preventive care rewards? All UPMC for Life members have an opportunity to earn a $30 reward for completing an annual wellness visit. Use this time to create or update your personal health care plan with your doctor. You may also be eligible to earn rewards for the preventive care below if you meet the following criteria. UPMC for Life will notify you each year if reward opportunities are available to you. Preventive care rewards are determined annually and may change each year based on your health care needs. * A separate copay may apply if additional medical services are received during the same visit or if your screening becomes diagnostic in nature. Delivery time frames may vary and are based on when your provider submits your preventive care claim.
