Privacy Statement UPMC Health Plan is committed to keeping your personal information private and con dential. This is a commitment that we take seriously. Whether you are a prospective, current, or former member, living or deceased, we respect and safeguard the privacy and con dentiality of your information. Privacy is one of your rights as a consumer as well as a UPMC Health Plan member. It also is a right that you retain even when you are no longer a member of UPMC Health Plan. But what does “private and con dential” really mean? When you complete your enrollment or renewal application form and sign it, you are doing two important things: 1. Verifying that the information that you have provided to us is correct and truthful. 2. Acknowledging that UPMC Health Plan can use the information we collect or receive about you and your family only for very well-de ned purposes. Your personal information covers a number of elements that all have one thing in common: They are unique to you and they can be used to identify you. This means that any les containing information that includes things such as your name, address, Social Security number, and birth date are considered “protected health information” (“PHI”). It is our responsibility to ensure the privacy of the PHI of all our members—prospective, current, or former. The les that UPMC Health Plan collects or maintains include things such as the claims we have received and paid, the services provided to you, or the health care premiums that you or your employer have paid. UPMC Insurance Services Division participates in the ClinicalConnect Health Information Exchange (“ClinicalConnect HIE”). The individual PHI and health information stored within ClinicalConnect HIE includes data such as test results, medication lists, consultation and progress notes, and clinical claims information. UPMC Insurance Services Division data will be shared only if the member has been to a ClinicalConnect HIE provider or facility and, while there, did not choose to opt out of such information sharing. Members must notify their ClinicalConnect HIE provider if they do not wish to participate in the HIE. UPMC Insurance Services Division and UPMC established an Organized Health Care Arrangement (“OHCA”) to conduct analysis for quality assessment and improvement activities, utilization review, payment activities, and clinical solutions development to facilitate more effective and ef cient delivery of health care services to patients and members.
