Coverage when you travel Travel Concierge Program HMO members have in-network coverage while traveling in Arizona, Florida, Georgia, North Carolina, South Carolina, and Tennessee. 3 You pay the same cost sharing when seeing a provider in these states as you would when seeing an in-network provider at home. Call your Health Care Concierge team before you see a provider while traveling so that we can help coordinate your care. Your Health Care Concierge team can help you: • Locate a provider or pharmacy. • Schedule a medical appointment. • Get prior authorization for services, as needed. Remember that you cannot be out of the service area for six continuous months out of the year. If you are away for six months or longer, you will be disenrolled from your UPMC for Life plan. Worldwide emergency assistance Assist America provides help 24/7 when you travel more than 100 miles away from home, including to another country. This program connects you to doctors, hospitals, pharmacies, and other services all over the world so you’re never without access to care. This benefit is included with your membership at no additional cost to you. Assist America provides help with: • Emergency medical evacuation. • Hospital admission assistance. • Medical monitoring. • Round-trip transportation for a family member or friend (if traveling alone). • Help replacing forgotten prescriptions (additional costs may apply). • Assistance with local language communications. • Member transportation if medically necessary. • Arranging for transport of mortal remains. To print an Assist America card, log in to My Health OnLine. Click on Your Insurance, then select ID Cards > Card Type > Assist America. 10
