14 Paying for your health care coverage Paying your monthly premium If you are enrolled in a plan with a monthly premium, you can pay your bill one of four ways: 1. Use autopay. Log in to My Health OnLine and click on Your Insurance, then select Premium Payments. 2. Call. Call the phone number on the back of your member ID card to make a payment over the phone or for help to set up autopay. 3. Visit. Come to a UPMC Health Plan Connect Center. Visit upmchealthplan.com/connect- centers for a complete listing of locations. 4. Send a check to: UPMC Health Plan PO Box 371842 Pittsburgh, PA 15250-7842 Cost of care Some UPMC for Life plans have a deductible, which is a preset dollar amount you pay for covered services before your plan begins to pay. Not all covered services apply to the deductible. • If a covered service does apply to the deductible, you must pay the total cost of the service until the preset deductible amount is paid in full. • If a covered service does not apply to the deductible, you pay a copay or coinsurance. When you receive care, you will typically pay a copay to the provider. A copay is a fixed amount you pay for covered services. Your copay amount will depend on what type of doctor you are seeing and what type of visit you are having. If you have coinsurance, you will pay after you receive your bill. Coinsurance is a percentage of the cost that you pay when you receive covered services. For example, if your coinsurance is 20 percent, you must pay 20 percent of the cost of your health care services. UPMC for Life will pay the remaining 80 percent.
