SilverSneakers®

UPMC for Life Dual members can join a local participating fitness facility at no additional cost. The SilverSneakers® program can help you get physically fit and stay active. Here are some membership perks:

  • FREE gym membership to a participating fitness facility
  • Unlimited access to participating locations
  • One FREE personal training session per year at a participating fitness facility
  • Support from trained instructors
  • Exercise classes (where available)
  • Access to dance studios, yoga studios, and swimming pools (where available)
  • Amenities such as saunas, steam rooms, and whirlpools (where available)
  • Website support at www.SilverSneakers.com (includes on-demand workout videos plus health and nutrition tips)
  • The option to work out at home with SilverSneakers Steps®

You can choose to work out in the comfort of your own home instead of joining a fitness facility. With SilverSneakers Steps®, you can choose two of these Home Fitness kits: Cardio Strength, Walking, Stress Relief, and Yoga. Kits are mailed to you and may include a DVD, booklet, and Quick-Start guide.

Bathroom safety products

UPMC for Life Dual members can select up to six different bathroom safety products from a menu of items. The bathroom safety kit includes safety products such as a transfer bench, a hand-held shower head, bath mats, and raised toilet seats. This is available to members at no cost. UPMC for Life is not responsible for the assembly or installation of the products included in the bathroom safety kit. Prior authorization is required.

In-home safety assessment

UPMC for Life Dual members can get one FREE in-home safety assessment each year with a trained home health provider. You’ll get tips and advice to help you live independently and safely at home.

Personal counseling services

UPMC for Life Dual members have a $0 copay for six counseling sessions every year. You’ll work with a trained and licensed counselor.

Meal service

UPMC for Life Dual members can receive home-delivered meals after they are discharged from an inpatient hospital stay, observation stay, or a skilled nursing facility stay. This benefit is included in your plan at no additional cost to you. It includes 14 meals delivered to your home. We’ll work with you to coordinate your first week of meals when you’re back home so you don’t have to. Prior authorization is required.

Assist America Travel Assistance

Assist America provides help when you travel more than 100 miles from home or even to another country. Help is available 24 hours a day, 365 days a year, anywhere in the world. Assist America provides these services:

  • Emergency medical evacuation
  • Hospital admission assistance
  • Medical monitoring
  • Round-trip transportation for a family member or friend (if traveling alone)
  • Help replacing forgotten prescriptions (additional costs may apply)
  • Assistance with local language communications
  • Member transportation if medically necessary
  • Arranging for transport of mortal remains

Assist America does not replace your UPMC for Life Dual emergency benefits. If you have an emergency, go to the nearest hospital or urgent care facility. You will be covered under your UPMC for Life medical coverage.

UPMC MyHealth 24/7 Nurse Line

As a UPMC for Life member, you can call the UPMC MyHealth 24/7 Nurse Line for health advice 24/7. You can get general health information or facts about a specific medical issue. Experienced, registered nurses are on hand 24/7 to provide you with prompt and efficient service.

UPMC nurses who answer calls are licensed to assist members located in Pennsylvania, West Virginia, and Ohio. Members must be located in one of those states when calling the UPMC MyHealth 24/7 Nurse Line.

UPMC AnywhereCare

UPMC AnywhereCare gives you access to low-cost, high-quality care for common medical conditions through a virtual visit. UPMC AnywhereCare providers* are easily accessible over live video straight from your smartphone, tablet, or computer – anytime, day or night for a $0 copay. The service works best for conditions such as colds and flu, sinus infections, coughs and sore throats, urinary symptoms, back pain, sunburn, pink eye, and poison ivy.

*UPMC Health Plan members located in Pennsylvania at the time of the service will have a virtual visit with a UPMC-employed provider. If a member is located outside Pennsylvania, the service will be delivered by a separate provider group — Online Care Group (OCG). For dermatology e-visits, members must be located in Pennsylvania during the online visit.

MyHealth OnLine

MyHealth OnLine is your one-stop destination for health care tools. This secure member website allows you to view your plan benefits. You also have access to health and wellness tools, your plan benefits, claims, and much more. Once you become a UPMC for Life member, we’ll send you information on how to log in and get started.

Care Management Programs

UPMC's care managers are here to help you manage chronic medical conditions such as heart disease, asthma, diabetes, and high blood pressure so that you can feel your best.

Complex Case Management

If you have a complex medical or behavioral health condition, we can provide resources to help you stay as healthy as possible. You can talk to a health coach who will work with you, your caregivers, and your doctors to develop a plan that meets your needs. Call a health coach toll-free at 1-866-778-6073 Monday through Friday from 7 a.m. to 8 p.m. and Saturday 8 a.m. to 3 p.m. TTY users should call 711.

UPMC for Life has a contract with Medicare to provide HMO, HMO SNP, and PPO plans. The HMO SNP plan has a contract with the PA State Medical Assistance program. Enrollment in UPMC for Life depends on contract renewal.
