Get rewarded for getting the right care for you As a UPMC for Life member, you can earn a $30 reward for completing your annual wellness visit by Dec. 31. Your annual wellness visit gives you the time you need to catch up with your doctor on your health care needs. Below is a list of questions to think about before your visit. This can help you have a better conversation with your doctor. Easy access to care through a telehealth video visit Many doctors now use telehealth video services for annual wellness visits and other nonemergency health care needs. A telehealth visit allows you to see your doctor face-to-face through video chat. All you need is a smartphone, tablet, or computer with a camera and microphone. Talk to your doctor to see if telehealth is right for your specific health needs. If so, your doctor’s office can help you schedule your appointment and walk you through what to expect next. For questions about the preventive care rewards program or to see if you’re eligible for rewards this year, please call your Health Care Concierge team at the number on your member ID card. Do you have a complex medical or behavioral health need? Do you want to join a program that can help you manage a health condition like diabetes, asthma, chronic obstructive pulmonary disease (COPD), depression, or anxiety? Our care managers can help you determine which program is best for you. If you join our complex case management program, a health coach can: • Work with your doctors. • Help you get the health care services you need. • Help you understand your treatment plan. • Teach you and your caregivers how to manage your condition. • Tell you about community services that may be able to help you. • Determine your benefits and what is needed to support your treatment plan. • Develop a plan to help you manage your condition on your own. If you join a health management program, a health coach can: • Teach you about your health condition and how to manage it. • Give you tips for sticking to a diet that is right for you. • Encourage you to take your medicine(s) as directed. • Help you keep your doctor appointments. • Call to talk to you about your health, diet, and medication(s). • Talk to your doctor to let the doctor know how you are doing (with your approval). Staff members are available Monday through Friday from 8 a.m. to 5:30 p.m. Participation in these programs is not required, and you can stop at any time.
