Network and access
As a PSHB member with UPMC Health Plan coverage, you have access to the top-ranked doctors of UPMC. Nearly 500 UPMC physicians have been recognized as 2025 "Top Doctors" across more than 60 different specialties. These physicians were part of the Top Doctors® list, which was compiled by Castle Connolly. The Castle Connolly Top Doctor selection process is entirely merit-based. Doctors cannot pay to be listed.
You also have access to an extended network of providers for care wherever you are in the U.S.,4 as well as access to more than 65,000 pharmacies nationwide.
Medicare options
You can get more out of your coverage by combining a Medicare plan option with our PSHB plan options. You can lower your out-of-pocket costs when you pair the Standard Option HMO with our Medicare Advantage plan: UPMC for Life PSHB Retirees (HMO). You can also pair your Original Medicare with the Standard Option HMO.

1095-B tax forms
As of January 2020, the Internal Revenue Service no longer requires you to file the 1095-B form as part of your federal tax return because Congress eliminated the tax penalty for failing to meet the individual mandate. UPMC Health Plan will no longer mail the form to you, but you can easily obtain an electronic copy by visiting the UPMC Health Plan member site for quick and convenient access. Search under Forms and Guides, then navigate to Plans and Coverage.
You may also request a printed copy of your 1095-B form by:
- Mailing your request to Attn: Enrollment, UPMC Health Plan, 600 Grant Street, Pittsburgh, PA 15219.
- Submitting a request online by filling out our 1095-B contact form or emailing your request to 1095BInquiry@upmc.edu. Please include your member ID, name, and the tax year being requested.

1UPMC Health Plan members who are in Pennsylvania at the time of a virtual visit may select a UPMC-employed provider, subject to availability and discretion of the provider. Members located outside of Pennsylvania at the time of service or those who select Talk Therapy or Psychiatry services will receive care from a provider employed or contracted by Online Care Network II PC (OCN), also known as Amwell Medical Group. OCN is not an affiliate of UPMC. Limitations may apply for members of ASO plans that have opted out of coverage. Providers are not available to treat members who are in Puerto Rico.
2UPMC nurses who answer calls are licensed to assist members in Pennsylvania, West Virginia, Maryland, New York, and Ohio. Members must be in one of those states when calling the UPMC MyHealth 24/7 Nurse Line. The UPMC MyHealth 24/7 Nurse Line is not a substitute for medical care. If an emergency arises, call 911 or go to the emergency department. Nurses cannot answer plan or benefit questions. Please call the Member Services phone number on the back of your ID card for questions regarding your plan benefits.

4UPMC Health Plan commercial members and their dependents who reside in Ohio do not have access to the Cigna Healthcare PPO Network and will access care through the SuperMed PPO Network. Ohio residents/members, while traveling outside of the state of Ohio, can use the Cigna Healthcare PPO Network.
