Transition of Care

At UPMC Health Plan we believe there’s nothing more important than your health and the health of your family. That’s why we offer coverage of the highest quality and value to you every day.

Before switching to UPMC Health Plan, you may be receiving ongoing services from physicians or other health care providers who are not part of the UPMC Health Plan network. Fortunately, we make transitioning your care easy.

If you join UPMC Health Plan while receiving ongoing treatment from a health care provider who is not in our network, you may request transition of care coverage. If approved, this coverage may allow you, under certain circumstances, to continue receiving care from your existing provider for a limited period of time. Transition of care requests must be submitted within 30 days from the beginning of your UPMC Health Plan coverage.
