Once you return this completed, signed, and dated form to us, we can verify your request, adjust our records accordingly, and speak to your personal representative.

MyUPMC

Get care without leaving home! With MyUPMC, you can schedule an online visit with your regular provider. All you need is your phone or tablet—and the MyUPMC app. Plus, you can track your health care and review information about the care you’ve received from your UPMC providers.¹
