This suite of services lets you work with nurses, dietitians, and other professionals to manage your health conditions and make healthy lifestyle changes. Together, you’ll create a customized plan to achieve your personal health goals and work through challenges along the way.

If you want to lose weight, be more active, or stop smoking, our health coaches can help. If you have a health condition and want to feel better by learning to manage it, our care managers have the experience to handle your needs.

Your health coach or care manager will use proven techniques to help you get the results you want. They will get to know you and help you find out if anything is holding you back from succeeding. If you’re already working to better manage a condition, they can help you step up your efforts. They can also direct you to community and online resources. But you are the leader of your team. You choose how you want to work with us—as much or as little as you want.

Everything we do supports the care plans that your provider develops. Our lifestyle and care management plans are personalized for and with you. We discuss what you want to talk about, and we can have a session with you as often as you wish. You don’t have to make an appointment or travel to an office. If your provider prescribes you to work with our team, we will create a progress note and share it back. This way, your provider will know about all the progress you’re making!
