Health Coaching and Condition Management

Our members contact UPMC Health Plan health coaches for help making healthy changes in their lives. They get more than results; they get to know a supportive, knowledgeable professional who's in their corner. Most of us have an idea of what we need to do in order to lose weight, become more active, or make changes to improve a health condition like diabetes or heart disease. We just don’t do it. What we really need to become healthier is sustained motivation.

That’s where a health coach can help. You can sign up for health coaching for free as a UPMC Health Plan member. You can meet with your health coach as often as needed by telephone. He or she will give you plenty of advice and practical information. But health coaches are especially good at inspiring you to take that first step to better health. And the step after that. And the step after that.

Your health coach can help you:

  • Come up with a clear goal and create a plan to get there.
  • Make changes that last.
  • Stay motivated.
  • Keep your goal in sight.
  • Overcome obstacles.

What’s the secret to success? Our health coaches use a proven technique called motivational interviewing. This method helps you determine what truly inspires you to become healthier. Once you have that focus, your journey to better health gets much easier.

If you’re living with a health condition, we can make things easier for you by helping to coordinate your care. Our health coaches can help you follow your doctor’s care plan, explain your treatment options, put you in touch with community resources, and help you improve your skills so you can better manage your condition.
