  • Gaining awareness that supporting diagnosis codes in the medical record also supports the level of medical decision making that in turns drives E&M code assignment.
  • Reviewing common provider diagnosis documentation deficiencies in the E&M record.
  • Demonstrating an understanding of official documentation and coding guidelines for proper ICD-10-CM code reporting.
  • Building skills of best practice documentation to support ICD-10-CM diagnosis codes.
