Explanation of Benefits (EOB)


What your EOB can tell you

An EOB is a document you receive after a health care visit. It lets you know the type of visit you had and how much you have already paid or may still owe for the visit. It is not a bill.

We know the EOB can be confusing. That’s why we’ve updated our EOBs to make them easier to understand. Here’s a quick guide to help you navigate your new EOB. Click the image below or view the sample EOB (PDF) to get started.

Save time, space, and the environment by going paperless!

You can now choose easy and quick access to your EOB digitally. Instead of sending your EOB in the mail, we will email you a link to it in MyHealth OnLine, your secure member website. All your EOBs are stored and available to you in MyHealth OnLine in case you need them.

Ready to go paperless?

Simply log in to MyHealth OnLine to update your preferences.

Already a MyHealth OnLine user?

  • Log in to MyHealth OnLine.
  • Go to the Menu in the upper left corner and choose Preferences.
  • Click the toggle to change your EOB preference. When you see a checkmark, you’ve selected the option to receive your EOBs digitally.

New to MyHealth OnLine?

View your EOBs on MyHealth OnLine anytime

  • Log in to MyHealth OnLine.
  • On the homepage under Your Insurance, click Claims and EOBs.
  • Here you can sort through, view, and download your EOBs.

If you receive your EOBs in large print or a language other than English, you will still receive a paper copy.

UPMC for Life has a contract with Medicare to provide HMO, HMO SNP, and PPO plans. The HMO SNP plans have a contract with the PA State Medical Assistance program. Enrollment in UPMC for Life depends on contract renewal.
