The UPMC Health Plan HIPAA Fax Line 412-454-7829 has been permanently closed and is no longer receiving faxes. The most efficient way to submit a PRD (Personal Representative Designation) form is for the member to do so via the Member Site (Digital Account).

Completing a PRD Form on the Member Site is much faster and more efficient than sending in a paper PRD form; however, if the member needs to submit a paper request, please submit directly to:

When mailing HIPAA documents, please include the member’s name, DOB, and/or Member ID on each page of your document. This helps us ensure we have all the pages of a document, and that those documents will be added to the member’s account. Processing may be delayed if we do not have this information and are unable to locate or confirm the member in our systems.

Provider OnLine Multi-Factor Authentication Enhancements coming soon

Microsoft changes and security enhancements will soon require Provider OnLine users to complete multi-factor authentication (MFA) each time they log in to access the portal. MFA is a security feature that adds an extra layer of protection by requiring more than just a password. This MFA requirement is expected to take effect on June 25, 2026.
