IMPORTANT NOTICE: The Biden Administration has announced that the COVID-19 PHE is expected to end on May 11, 2023. Some benefits or flexibilities made available during the PHE will expire on that date. UPMC Health Plan will update this Appendix A with more information about expiring benefits prior to the end of the PHE.

Pulmicort Flexhaler has a new distributor, Rubicon Holding. National Drug Codes (NDCs) for Pulmicort Flexhaler from Rubicon Holding are not included in the Medicaid Drug Rebate program and are not covered under the Medicaid Prescription Drug Program.
