Prescription for Wellness is a nationally recognized, Six Sigma-developed best practice system that provides a convenient and effective way for providers to prescribe resources and support offered by UPMC Health Plan to their patients who are UPMC Health Plan members.

At the end of this course attendees should be able to:

  • Describe strategies they can deploy to integrate care across payer and provider with the goal of improving population health outcomes and addressing health care disparities.
  • Explain how to utilize quality measures and data analytics to evaluate the impact of provider-prescribed health coaching and care management on patient-member health, self-care for chronic diseases, and shared decision-making.
  • Define best practice processes to promote strategic implementation and optimization of a patient-member engagement platform.
