Please be advised of the change effective Sept. 20, 2022, to authorizations regarding the following heart catheter code(s):
  • Right heart catheter CPT 93456 no longer requires an authorization.
  • Left heart catheter CPT 93452 no longer requires an authorization.

Unique Pharmaceutical Laboratories (A Div. of J. B. Chemicals & Pharmaceuticals Ltd.) is recalling four lots (GY825029, GY825030, GY825031, GY825032) of Cetirizine Hydrochloride Tablets USP 5 mg. Cetirizine is used for the relief of allergy symptoms, including itchy, red, watery eyes and itchy skin.
