2025 Preventative Care Rewards Program

Feb. 2025What's New

We’re excited to continue our preventive care rewards program in 2025. This program allows eligible UPMC for Life patients to earn rewards when they complete certain preventive services, such as an annual wellness visit, a breast cancer screening, a colorectal cancer screening, or a diabetic retinal eye exam (for members with diabetes only).

UPMC for Life patients can earn their reward by completing their annual wellness visit either in person or through telehealth. Talk to your patients to determine if this is a good option for them based on their health needs. Go to upmchp.us/telehealthguidelines - Opens in new tab to see our UPMC Health Plan’s Provider Telehealth Toolkit for resources that may help when discussing this type of care with your patients.

You can learn more about the 2025 preventive care rewards program at upmchp.us/MCSNPIncentive - Opens in new tab

We hope you will encourage your eligible1 patients to participate. If you have any questions, please contact your physician account executive or call Provider Services at 1-866-918-1595.

1Please consult the program materials to confirm eligibility requirements.

UPMC for Life is a product of and operated by UPMC Health Plan Inc., UPMC Health Network Inc., UPMC Health Benefits Inc., UPMC for You Inc., and UPMC Health Coverage Inc.
