Mandatory training for Medicare SNP providers

The Centers for Medicare and Medicaid Services (CMS) require all Medicare Special Needs Plan (SNP) providers to complete the Model of Care (MOC) Training every year. The MOC describes the structure and process by which health care services are delivered to SNP members.
For your convenience, UPMC Health Plan has developed a brief online training module on the MOC for our SNP product, UPMC for Life Complete Care (HMO-SNP) (formerly known as UPMC for Life Dual).
To complete the training, go to upmchp.us/snpmoc You will need your tax identification number to sign in and need to attest to completing the training.
Please note that this training must be completed by Feb. 29, 2024.
