Home-delivered meals

UPMC for Life Complete Care (HMO SNP) members can get home-delivered meals after an inpatient hospital stay, observation stay, or a skilled nursing facility stay. You can receive a single delivery of 14 meals to your home at no cost. Meal options are healthy and can be custom-made to your dietary needs.

A care manager may visit you while you are in the hospital or a skilled nursing facility. Or, after you are discharged, you will receive a call from one of our care managers to schedule your meal delivery. We’ll work with you to coordinate your first week of meals when you return home so you can focus on your recovery.

UPMC for Life has a contract with Medicare to provide HMO, HMO SNP, and PPO plans. The HMO SNP plans have a contract with the PA State Medical Assistance program. Enrollment in UPMC for Life depends on contract renewal.
