Our members have access to personal programs and resources that can help you coordinate your care, make healthy lifestyle changes, and even provide support for serious illnesses.

Health Coaches
A UPMC Health Plan health coach can help you learn how to:
- Quit smoking.
- Lose weight.
- Eat healthier.
- Manage stress.
- Become more active and physically fit.
- Get assistance with care coordination.
- Manage diabetes, cardiovascular health, and respiratory issues.
Members can take advantage of health coaching by calling us toll-free at 1-855-395-8762 (TTY: 711) Monday through Friday from 8 a.m. to 4:30 p.m.
Personal Care Managers
A personal care manager can help you:
- Connect with the best doctors for your needs and coordinate your care.
- Understand your doctor’s instructions after a visit.
- Contact your doctor’s office if a prescription is not filled or a service is not covered, and let you know ways to save money on your medications.
- Find services in your community that you may need.
Members can call us toll-free at 1-855-395-8762 (TTY: 711) Monday through Friday from 8 a.m. to 4:30 p.m. to get started.
Support for serious illness
If you have a serious or advanced illness, you can get extra care and support at no extra cost. This benefit is for members living at home or in an assisted living facility. It is not available to members in a skilled nursing facility.
Our care team can:
- Create a personal care plan that meets your needs.
- Help you manage your symptoms and medications.
- Deliver meals to your home, if needed.

Personal Counseling
You can receive six counseling sessions per concern per year—over the phone or through a virtual visit—at no extra cost.* Your counselor can help you with family and relationship concerns, stress, grief, and other emotions you may be experiencing. Call 1-866-441-4395 (TTY: 711) Monday through Friday from 8 a.m. to 5 p.m. to get started. You can also connect with a counselor face-to-face through UPMC AnywhereCare. Just select Virtual Counseling to schedule your first session after you sign up or log in.

Resources for Caregivers
Counseling for caregivers
Your family members or caregivers can receive six counseling sessions at no extra cost.* This benefit can be used to help caregivers reduce their feelings of loss, loneliness, or stress. It can also offer caregivers resources for communicating effectively, making tough decisions, setting goals, and solving problems. Call 1-866-441-4395 (TTY: 711) Monday through Friday from 8 a.m. to 5 p.m. to get started.
Powerful Tools for Caregivers
This six-week online class teaches you how to take care of yourself while caring for a spouse, relative, or friend who lives at home or in a nursing home. It is available to members and all attendees at no cost. For more information, call your Health Care Concierge team at 1-877-539-3080 (TTY: 711).
