Healthy Food Kits

UPMC for Life Complete Care (HMO D-SNP) members who qualify for the Special Supplemental Benefits for the Chronically Ill (SSBCI) program can order two healthy food kits per year! Qualifying members can order one kit between Jan. 1 and June 30 and one kit between July 1 and Dec. 31.

If you’re already a UPMC for Life Complete Care member and qualify for the SSBCI program, here are three easy ways to order your healthy food kit starting Jan. 1, 2026:

• Call 1-888-356-5009 (TTY: 711) seven days a week from 8 a.m. to 8 p.m. Have your UPMC for Life member ID card and date of birth ready.
• Log in to the UPMC Health Plan member site. Click Wellness at the top of the homepage, then click Healthy Food Kits. 
• Log in to the UPMC Health Plan app. Tap the Wellness tab, then tap Healthy Food Kits.
