Most preventive services are available at no cost to members when you see an in-network doctor.* Plus, UPMC for Life Complete Care (HMO D-SNP) members can earn rewards for completing certain preventive care and taking steps to maintain their best health.

Preventive care reward program

The UPMC for Life Complete Care preventive care reward program allows members to earn reward dollars for completing certain preventive screenings by Dec. 31 each year. Once you become a member, we’ll let you know which preventive screenings you are eligible for during the year.

Annual wellness visit—$30 reward
Getting an annual wellness visit is one of the very best things you can do for your health. This important visit gives you the time you need to catch up with your doctor and create or update your personal care plan. We encourage you to reach out to your doctor and ask about your options for completing your annual wellness visit. Many doctors offer telehealth video visits, while others may prefer you come into the office. This is based on your needs and should be discussed with your doctor.
Diabetic retinal eye exam—$25 reward
Breast cancer screening—$50 reward
Colorectal cancer screening—$60 reward
Health assessment with an eligible provider—$25 reward
