We want to make it easy for you to get the health care services you need. That’s why the UPMC for Life Complete Care (HMO SNP) plan offers trips to plan-approved locations at no additional cost. The number of trips your plan offers depends on where you live.

Members can schedule trips to places like the doctor’s office, the hospital, health care facilities, the pharmacy, fitness centers, grocery stores, adult day and community centers at no cost.

Using the transportation benefit is easy. You, a family member, or your caregiver can call to schedule your trip. If you need a wheelchair van or help to get into the vehicle, let the transportation service know when you call. You will receive a confirmation call 48 hours before your appointment to confirm your trip.
